NEW PATIENTS

 

Appointments


Appointments can be made by phone for new patients. For established patients, you should schedule your next visit at the time of your last appointment.  By letting the receptionist know the nature of your appointment, she can schedule you for the proper amount of time and reduce the number of return visits.  If you need to cancel an appointment, we require at least 24 hours notice.  Without 24 hour notice, there will be a missed appointment fee.

 

Insurance and Fees


ARDA participates with Medicare, Anthem Blue Cross/ Blue Shield and several other insurance plans.  Participation may vary as changes occur within the insurance industry.  Please check with our receptionist to find out if we participate with your insurance plan.

 

For you convenience, we will file your primary insurance claims if you provide us with correct billing information.  We ask that you pay your deductible, co-insurance and any co-payments at the time of your office visit.  Cash, check, VISA and MasterCard are accepted.

New Patient Forms

 

You may fill out your new patient or Osteoporosis Evaluation Center form at the time of our visit and we recommend that you arrive 30 minutes prior to your scheduled visit to allow enough time to complete the form and check in.  Alternatively, you can click on the links below to download the appropriate form which you can complete at your leisure and send to ARDA via fax (703-425-4495), mail or in person at your scheduled visit.
